Pedro Medina burst onto the scene in 1904 looking like a star and is out of the league by 1908, in what is as clear an example of career statistics pointing to long-term injury as you can find.
He is great as a 20 year old rookie, then excellent under a little lighter workload in 1905, then essentially unusable.
When good, Medina was hard to hit with excellent control, although his K rate was never as high as perhaps necessary for true stardom. Still, being in the running for Rookie of the Year in 1904 and contributing to a top tier rotation that year and the following are well within the realm of possibility.
And then it all falls apart, with essentially no explanation. He is easy to hit in 1906, and while the control is still there, the stuff has gone and after a very few innings in 1907, Medina calls it quits.
